An email address is required for registration confirmation and to access the webcast.  Please complete ALL required sections.

For webcast registrants:  Once your registration is processed, a confirmation and your webcast login information will be emailed.  To earn CEU credits, your CCA number must also be entered on the registration page.  Test the login site to become familiar with the interface and bookmark the location so you can return for the live webcast on Wednesday, December 12 and Thursday, December 13, 2012.

Early-bird registration fee = $150

Early bird registration deadline is December 5, 2012 at 11:59 pm.
Registration deadline is December 9, 2012 at 11:59 pm.

Cancellations: $25.00 non-refundable until November 30.
No refund after November 30, 2012.
