To submit a form, you can:

EMAIL: Complete the form electronically and then submit it by email to the Arts General Office.   

MAIL: Complete the form electronically then print it, or print the form and complete it by hand, and mail it to the address at the top right of the form.

IN PERSON: Come to the Arts General Office (3rd Floor Fletcher Argue Building) and complete the form in person. .
