Adult Criminal Record Self Declaration Statement, Official Adult Criminal Record Search, A Child Abuse Registry Statement, And Child Abuse Registry Record
Extended Education undertakes to ensure that all persons admitted to the division, and the program, have not engaged in behavior that is regarded as physically and mentally harmful to others. In this regard, all applicants offered admission will be required to indicate whether or not they have an adult criminal record and/or whether they are listed on the Child Abuse Registry Record.

- Adult Criminal Record and Child Abuse Registry Self-Declaration
All applicants are required to submit a self-declaration, indicating whether or not they have an adult criminal record, and a child abuse registry record. The self-declarations can be accessed on-line under Admission Requirements on the Applied Counselling home page The self-declarations must be completed and returned immediately to the address below.

Failure to disclose any adult criminal record and/or child abuse registry record will invalidate an applicant’s application and result in automatic expulsion from the program if the applicant has been admitted.
- Official Adult Criminal Record Search and Child Abuse Registry Search
Please apply for these documents early as the process takes a few weeks. The Official Adult Criminal Record Search and the Child Abuse Registry Search are required as a condition of graduation from the program must accompany the Application for Graduation Note: A juvenile record does not constitute an adult criminal record.
- An Official Adult Criminal Record Search can be obtained from the following:
- Winnipeg Applicants can obtain an application at the Bureau of Police Records, 4th floor of the Public Safety Building, 151 Princess Street, or by directly phoning 986-6074, or http://winnipeg.ca/police/pr/info_request.stm.
- Brandon Applicants can obtain an application through the City of Brandon Police Service, 1020 Victoria Avenue, Brandon, Manitoba, R7A 1A9, 204-729-2345 or http://police.brandon.ca/
- Out-of-province Applicants can apply through their home jurisdictions.
Note: For the above, two pieces of identification are required.
- A Provincial Child Abuse Registry application can be obtained from the following:
- The Child Abuse Registry Unit, 201-114 Garry Street, Winnipeg, Manitoba R3C 4V5. Phone (204) 945-6967 or http://www.gov.mb.ca/fs/childfam/child_abuse_registry_form.html. Allow up to six weeks for processing.
- The applicant will be responsible for the costs associated with providing the Official Adult Criminal Record Search and a Child Abuse Registry check, and for ensuring that the statements are submitted by the required deadline.
- Applicants must submit all documents to the above-listed address by the deadlines specified.
