Evaluation Procedures & Forms

It is the expectation of the SEO that Teacher Candidates seek and receive formative feedback throughout the practicum.  The practicum is evaluated at the end of each block by a Faculty Advisor who works with the Teacher Candidate and the Cooperating Teacher to review the practicum and compile a Summative Evaluation Report. The Summative Evaluation is based on the Practicum Expectations.

Evaluating the practicum is an ongoing process that involves a variety of approaches.
  Procedures for At-risk Teacher Candidates (pdf)
When Teacher Candidates are at-risk of failing the practicum there are a variety of supports available and  procedures to follow.

As noted in the Procedures for At-risk Teacher Candidates, a Notification of Concern is to be filed by the Faculty Advisor when a Teacher Candidate is experiencing difficulty in the practicum and may fail.

Collaboration is a critical aspect of teaching. All Teacher Candidates should invite a peer to evaluate their teaching.

Formative Evaluation Form

The Formative Evaluation form is to be completed by the Faculty Advisors and/or the Cooperating Teacher (CT). Formative Evaluation is an opportunity to provide the Teacher Candidate (TC) with support and feedback in regards to their professional development.
