It is the expectation of the SEO that Teacher Candidates seek and receive formative feedback throughout the practicum. The practicum is evaluated at the end of each block by a Faculty Advisor who works with the Teacher Candidate and the Cooperating Teacher to review the practicum and compile a Summative Evaluation Report. The Summative Evaluation is based on the Practicum Expectations.

Formative Evaluation Form The Formative Evaluation form is to be completed by the Faculty Advisors and/or the Cooperating Teacher (CT). Formative Evaluation is an opportunity to provide the Teacher Candidate (TC) with support and feedback in regards to their professional development.
