All returning players must fill out the annual health reassessment questionnaire and all new players must fill out the medical release form and have the pre-season medical questionnaire completed by a doctor. All forms can be downloaded at the following link:

If you require a doctor to complete the physical contact Dr.Podolski at Skylark Medical Clinic at (204) 453-9107 and state you need a physical for Bison Women’s Hockey Fall Camp.
This year’s camp will be intense with various measures designed to test you on and off the ice including the beep, on-ice fatigue and various other tests designed to measure your fitness and observe your character. Ensure you have done the work necessary to compete for a spot by challenging yourself this spring and summer with a hockey specific training program.
