Assessment Process

The main steps in the assessment process include the following:

1. MPAP Program Director receives a candidate referral from the CPSM.
2. MPAP Program Team reviews the candidate referral.
3. Candidate Package is forwarded to the candidate from the MPAP Office.
4. Candidate completes and forwards back to the MPAP Office all required hardcopy
and online documentation,  distributes Patient Survey to patients and pays the Administrative Fee. Note: Until Step 4 is completed, a candidate is NOT
 considered to be enrolled in the MPAP
5. MPAP Program Team reviews all candidate documentation.
6. Interview between the candidate and MPAP Program Team is booked and completed.
7. Assessors are selected.
8. Onsite assessment is scheduled after all Physician Colleague, Interprofessional Colleague, and Patient Surveys are
 received by the MPAP Office.
9. Assessment Fee (or portion thereof) is paid by the candidate prior to the onsite assessment.
Note: Payment plan options are available; however, the Assessment fee must be
paid in full prior to the MPAP Program Team forwarding the final assessment report to the candidate.
10. Onsite assessment is completed and assessors complete assessment documentation.
11. MPAP Program Team reviews assessment documentation and completes a final assessment report.
12. Candidate receives by mail a copy of the full assessment report from the MPAP Program
Team along with a copy of the  letter to be sent to the CPSM.
13. Candidate has 10 days to appeal the content of the final assessment
report on the basis of allegation of procedural  error only. The appeal must be made in writing (email acceptable) and forwarded to the MPAP Program Director. 
 Note: The candidate may request a meeting with the MPAP Program Director to discuss possible potential remediation  strategies; however, any remediation
undertaken by the candidate will not result in a change in the decision  regarding the current assessment.
The MPAP Program Director is not responsible for arranging remediation, but may be able to offer assistance. It is the responsibility of the candidate
to notify the CPSM if he/she plans to undertake another assessment should he/she be unsuccessful on the first attempt. The full cost of any additional assessment components
are paid by the candidate.
14. After the 10 day appeal period, the MPAP Program Team will forward a letter to the Registrar of the CPSM outlining  the decision(s) of the assessment process.
The final assessment report will not be forwarded to the CPSM unless  requested by the CPSM.
