Chamber Music Ensemble Form:
- All Small Chamber Music Ensembles are required to fill out this form and return it ASAP to Judy Kehler Siebert.
- Only one form is required per ensemble, but it must include all information for all members of the group.
- Please include details asked for and add any additional information or concerns at the bottom of the form.
- Submit by email only (no hard copies). This information will be used to confirm data of students' registration in this course.
