College of Nursing Practice: Declaration Process

Illness or other disruptive life circumstances can happen at any time that may result in missed clinical/lab time, course work or tests. When this happens, you must complete a Declaration within 48 hours if you are absent from or miss:

  • simulation,
  • clinical,
  • skills lab,
  • a term test, or
  • an assignment. 

Why do I Need a Declaration?

The university has policies in place to preserve the integrity of its academic programs and evaluation procedures. In cases where a student is absent or unable to meet their academic commitments due to these circumstances, the university will consider a Declaration in lieu of other documentation, such as a doctor’s note.

How do I Make a Declaration? (NEW)

  1. Download and electronically complete the Declaration Form.
  2. Complete a Declaration for EACH DAY you are absent.
  3. Email the completed form to and copy the applicable course leader(s). A Declaration must be submitted within 48 hours of the missed day.
  4. Include your student name and student number in the subject line of the email.
  5. Each Declaration is recorded electronically and filed.

Absenteeism is a concern within a professional practice program. The College of Nursing reserves the right to meet with the student and further discuss the situation around any absenteeism.

A false statement regarding a Declaration may result in disciplinary action, up to removal of a student from the Bachelor of Nursing program.
