Uninsured Services and Payment Options

UNIVERSITY HEALTH SERVICE (UHS) - UNINSURED SERVICES SCHEDULE

Fees For Uninsured Services as of January 1, 2012*  
 
UHS provides a number of medical and administrative services which are not covered by Manitoba Health or other provincial or independent medical insurance plans. These services will be billed directly to the patient as follows:
   
Sick Notes/Certificate of Fitness Forms  
• Students $25
• Staff $30
   
Forms
(disability services, pre-op, EI, etc.)		  
• Students $30

Narrative Letters
(e.g. authorized withdrawal, disability etc.)		 $75
(per page)
   
Transfer of Medical Records Fees range between $25.00 - $200.00 depending on the size of your medical records. Please contact the office for the fee.
   
Clinical Services  
Office Visit (15 min) $50
Complete Physical (30 min) $100
Driver’s License Exam and Form $130
Travel Consultation
(does not include the cost of any immunizations or injections needed)		 $100
Immunizations Given by
(vaccine supplied by client)		 $15
Mantoux Test given by RN
(includes injection of PPD [supplied by UHS] and reading of test 48-72 hours later)		 $30
Flu vaccines given by RN
(those not insured by Manitoba Health)		 $30
   
Missed Appointments  
15 minute appointment
(office visits, follow up or same day)		 $50
30 minute appointment
(complete physical or extended visits)		 $100
   
*Uninsured Services Fees in accordance with the Manitoba Medical Association’s approved Uninsured Services Manual February 2006.

FEES & PAYMENT

  • Please feel free to discuss these charges with your health care provider or other University Health Service staff member.
  • We accept payment in the form of cash or cheque.
  • We do not have interac or credit card capabilities.

HEALTH PLANS

  We accept all health plans whether from Canada or other health agencies including:
 
  • C.I.D.A. [Canadian International Development Agency]
  • Great West Life [most common for international students]
  • Canadian Commonwealth Scholarship/Fellowship Plan
  • CBIE [Canadian Bureau for International Education] and Seaboard Life Insurance
