Rady Faculty of Health Sciences Immunization Program

Students offered admission to a college, school or program within the Rady Faculty must maintain an up-to-date record of immunizations and tests as a condition of enrollment.

This includes students from the following areas:

All incoming students must:

• complete the immunization package
• read the student manual

These documents are provided to you by your program at the start of the school year. Please contact your program or our office If you require a copy.

If you are an incoming student in the nursing or midwifery program you must complete a different form provided to you by your program.
