Required documents

What is required?

As part of the admission process, all students entering a program in the College of Nursing are required to complete and submit:

  • Adult abuse registry check
  • Child abuse registry check
  • CPR
  • HSP net consent form
  • Immunization consent and student information forms
  • Police record check (criminal record check) including:
    • Vulnerable sector search

Costs

Students are responsible for the costs associated with the non-academic admission requirements.

Adult abuse registry check

Details on how to apply, costs and processing times are available on the Manitoba government adult abuse registry unit website.

Carefully review all instructions to ensure that your application can be processed promptly.

In jurisdictions where an adult abuse registry or equivalent does not exist, consult with the Student Services Office.

The original Manitoba adult abuse registry check is required, copies will not be accepted.

If your name appears on this registry, additional information will be required and you will be advised of the necessary next steps.

Please ensure you start this process early enough to meet the College deadlines. 

Child abuse registry check

Details on how to apply, costs and processing times are available on the Manitoba government child abuse registry unit website.

Carefully review all instructions to ensure that your application can be processed promptly.

The original Manitoba child abuse registry check documentation is required, copies will not be accepted.

In jurisdictions where a child abuse registry or equivalent does not exist, consult with the Student Services Office.

Any applicant on the child abuse registry will be denied admission.

Please ensure you start this process early enough to meet the College deadlines. 

CPR

Students must have Health Care Provider (HCP) or Basic Life Support (BLS) level certification. You may choose to certify anywhere in Canada, provided it is at the HCP or BLS level.

Some CPR cards state they are valid for more than one year. However, the College requires you to be certified no earlier than April 1 but must be certified by June 1 of the current year. 

Documentation can be submitted in the following forms:

  • Copy of the card
  • Picture of the card
  • Electronic print-out of the certification

Do not submit your original card.

Your documentation must indicate:

  • Your full name
  • Date certified
  • Level of certification

Returning students

All returning students, including those certified at the instructor level, must recertify each year to ensure that certification is valid for the entire year. Proof of recertification must be submitted to the Student Services Assistant or designate by the June 1 deadline.

HSPnet consent form

Sign the HSPnet consent form and submit with your non-academic admission requirements. We cannot enter any of your information into our system until this is complete.

Immunization consent and student information

All new students entering the Rady Faculty of Health Sciences must complete the immunization package, found on the Immunization Program page.

Previous immunization and testing records will be reviewed; students will be informed what additional items, if any, are required.

Missing immunizations and tests will be administered after admission.


Student immunization orientation modules

Upon registration, students will have access to the Rady Faculty of Health Sciences Student Immunization Orientation modules on UM Learn.

Police record check / criminal record check

IMPORTANT: Please ensure you start the check process early enough to meet the College deadlines. 


Residents of Winnipeg

Details on how to apply, costs and processing times are available on the City of Winnipeg website.

Step 1: Complete an online application for a police record check with vulnerable sector search.

  • Select ‘other’ for the reason you are applying
  • Select ‘University of Manitoba’ as the agency
  • Check the box that indicates you are requesting a vulnerable sector search

Step 2: You will receive an email with an attached PDF of your Criminal Record Check as well as a PIN code to access it. Please forward this PDF to nursing@umanitoba.ca.


Students from outside Winnipeg

If you are from a Manitoba location outside Winnipeg or from another province, you can apply for the criminal record check through your home jurisdiction or local RCMP.

If submitting a physical criminal record check, you must submit the original documentation. Copies will not be accepted.


International applicants 

You must obtain:

A Canadian certified criminal record OR police record check with a vulnerable sector search. Only police record checks / criminal record checks from the Winnipeg Police Services or the RCMP will be accepted.

AND

A criminal record check from your home country. International criminal record checks must be notarized and translated to English. If you are unable to submit a record from your home country, please complete and submit an affidavit. The affidavit must be signed and sealed by a notary public.

Mask fit testing is no longer an admission requirement and will occur as needed for clinical practice. 

Deadlines

Offers of admission are provisional based on the receipt of these documents. It may take from four to eight weeks to receive your documents once ordered, so please ensure you request them in advance of receiving your offer of admission. 

Your admission may be revoked if all documents are not received by the stated deadlines.

Submission

How to submit your documents

Provide all items as a complete package when you receive your offer of admission, ensuring that you retain a copy of any document you submit. 

We recommend that you either hand-deliver your documents to the drop box located at Reception in the Helen Glass Centre OR send by registered mail or courier:

We cannot verify receipt of documents. The College of Nursing is not responsible for lost or misdirected non-academic admission documents.

Processing

Please expect processing to take five (5) business days. You will be contacted should there be a problem with any of the documents submitted.

Return of documents

If you wish to have your documents returned, please provide a self-addressed and stamped envelope. Otherwise, the documents become the property of the College and we will have them confidentially destroyed when they are no longer needed.
